Coronary Radiography and Endarterectomy

چکیده
IN 1925, what was probably the first comprehensive clinicopathologic study of coronary heart disease was reported from this medical school by Benson and Hunter.' It is more than a quarter of a century later. Though the personnel have changed and though literally thousands of investigators have added to understanding of the problem, the basic concepts of coronary disease then elucidated have not been changed. Less fortunately, neither has the therapy. In view of progress in cardiovascular surgery,2the current state of angiography,9-16 and the frequency and mortality of coronary arterioselerosis17 it is now necessary to examine the feasibility of surgical removal of the intravascular obstruction. The present report describes the methods and findings of a continuing 3-year attempt to appraise the problems as well as the potential of such surgery. The planned objective was to use autopsy hearts in determining (a) whether the functional anatomy and carrying capacity of the diseased coronary bed could be improved by surgery, (b) whether coronary angiography offered reliable preoperative means for defining the lesion. In order to answer these questions, consideration had to be given to the severity and distribution of lesions as well as their physiologic and anatomic correlation with evidence of myocardial isehemia.

Early Clinical Outcomes and Mortality of Coronary Endarterectomy in the Left Anterior Descending Artery: A Single-Center Experience
Introduction: Although coronary endarterectomy is not an ideal procedure, it is the only available option for cardiac surgeons in some patients with diffuse coronary artery disease. Based on the majority of conducted studies, the results of coronary endarterectomy (including survival, graft patency, and recurrence of the symptoms of myocardial ischemia) are less prevalent than standard coronary...
متن کاملShort-term clinical outcomes and mortality of coronary endarterectomy in the right coronary artery: A single-center experience
Introduction: Coronary endarterectomy is not a perfect procedure. However, it is sometimes the only available alternative for cardiac surgeons in patients suffering from diffuse coronary artery disease. According to a large volume of studies, coronary endarterectomy results (such as survival, recurrence of the symptoms of myocardial ischemia, and graft patency) are not as satisfactory as standa...
متن کاملAnti-coagulation therapy following coronary endarterectomy in patient with coronary artery bypass graft
Background: Since there is a lack of research on postoperative anticoagulation protocol in patients undergoing coronary artery bypass graft (CABG) / coronary endarterectomy (CE), we recommend a new protocol for anticoagulation in these patients. Methods: In this double-blind randomized clinical trial study, 52 patients undergoing CABG / CE entered the study and were divided into two groups. In...

Introduction: Ischemic heart disease is the most common cause of mortality in developed countries. CABG (Coronary Artery Bypass Graft) is one of the therapeutic methods in ischemic heart disease. Methods and Materials: Considering the increased incidence of coronary artery disease in Iran, and with increased rate of CABG, we performed a cross sectioned study (1996-1999). In 635 patients 467 mal...
متن کاملReview on Coronary Endarterectomy - past , Present , and Future !
Coronary endarterectomy is first presented by Baily et al. as a surgical revascularization procedure against diffuse coronary artery disease on 29th October 1956. Coronary endarterectomy provides a way of complete surgical revascularization of the myocardium in diffuse and calcified coronary arteries with adequate blood flow to distal part of vessels, thus improving myocardial function. However...
